 Administrator PD Verification (1.2 MB) - This form is used by administrators to certify their professional development accrual.  This form and the artifacts of attendance should be taken to the summative conference and presented to the supervising entity.

 Credit for PD Request Form (7.82 KB) - This form is for anyone seeking ADE approval for a graduate class to count toward professional development. This form must be sent into the state prior to sending it to the district office. The school district authority is your building administrator. In the event you cannot locate him or her, a director's signature would suffice.

 Individual PD Development Record (767.35 KB) - This document is for teachers and administrators to log professional development hours annually. It provides the dates and times for easy recall.

 Supplemental PD Application (1.24 MB) - This form is for anyone seeking professional development approval for PD activities outside the district. This form must be submitted PRIOR to attendance of the event. The building level principal must approve it being sent to the district PD committee by signing the bottom of the form. If he or she approves, it then is sent to K. K. Bradshaw in the central office for committee vote.

 PD Evaluation (884.75 KB) - This form is to be used by schools or individuals to evaluate the professional development activities presented in the district.

 Presentations Application (626.3 KB) - This form is for those who are applying to present professional development activities in the district.

 Teacher PD Verification (878.22 KB) - This form is used by teachers to certify their professional development accrual. This form and the artifacts of attendance should be taken to the summative conference and presented to the principal.
